2020-2021 Oley Valley Lynx Athletic Registrations
For the 2020-2021 school year, the Athletic Department will be utilizing Student Central for athletic registrations and form submissions. This online registration platform will allow students and families in grades 7-12 to register for sports, complete all required athletic forms, and also confirm that you have completed all required steps in order for your child to start participation in a Fall, Winter or Spring sport.
Please take a moment to click on and read the following links listed below. These links will provide more information on how to sign your child up for any sport during the 2020-2021 school year as well as how to submit all required forms. It is recommended that the student create their account first, and then the parent can create their account and link them together by using a link that is created when the student creates his or her account. PLEASE MAKE SURE WHEN CREATING YOUR ACCOUNT, THAT YOU SELECT THE CORRECT SCHOOL. STUDENTS IN GRADES 7-8 SHOULD SELECT OLEY VALLEY MIDDLE SCHOOL AND STUDENTS IN GRADES 9-12 SHOULD SELECT OLEY VALLEY HIGH SCHOOL. Students registering for their first sport of the year will need to sign/initial sections 1, 3, 4, 5, and 10 and the Student-Athlete Code of Responsibility sections. Parents/Guardians registering for their child's first sport of the year will need to sign/initial all sections and forms. Students under the age of 13 are not currently required to sign all sections, but will be required to sign the concussion education section. Students and parents/guardians who are completing a re-certification for subsequent seasons will both need to sign/initial Section 7 and Section 10, and submit Section 8 if the student saw a doctor for an injury or illness during the previous sport season/s.
*Please note that while all forms are completed and submitted online, you will need to print out Section 5 (Health History) and Section 6 (Doctor's Physical Form) in order to be able to have a doctor complete the physical form. To print Section 5 and 6: While on the Section 5 page, click on the "Print/Upload Here" link that is found at the top of the Section 5 form. This will print all of your answers to Section 5 Health History as well as a blank Section 6 form to take to the doctor. Or you can also print any blank form by clicking on the "Athletic Forms" tab on the blue header bar and then scroll to the bottom and click on the "Example Forms, click here". Then select which blank form you would like to print out. A blank Section 6 form can also be downloaded and printed by clicking on the link at the bottom of this page, but most doctors will also required the completed Section 5 Health History form to be presented during the physical exam.
Registration is open for all students in grades 7-12 starting on June 1st. We ask that all students complete the online registration by June 30th. While there will be opportunity to change which sports your child signs up for leading up to each season, we ask that you try to complete the initial online process during the month of June.
Deadlines for all forms to be submitted on Student Central each season will be:
Fall Season - August 6, 2020
Winter Season - November 16, 2020
Spring Season - February 26, 2021

Section 6 - Doctors Form - can be printed to take to the doctor, and then must be uploaded to Student Central with the doctor signature - and dated after June 1, 2020.
*Per PIAA and School District Policy, all students must be covered by personal health insurance, or purchase the voluntary student accident insurance before participating in a scholastic sport.
Asthma Action Plan - To be completed each year by all student-athletes who have asthma - requires doctor certification. This form should be returned each year to the High School or Middle School Nurse or to the Athletic Office.
